Previous definitions have used absolute increases in SCr over baseline to define AKI ( Table 1 ). The implications of the decision to use absolute or percentage increases in SCr to define AKI have not to our knowledge been explicitly explored, particularly in the context of the time over which SCr rises.

SCr-based definitions of AKI

Presumably, defining AKI by higher SCr thresholds in patients with higher baseline SCr levels ( i.e. , chronic kidney disease [CKD]) is based on the well-described inverse correlation between creatinine clearance (CrCl) and SCr. If all determinants of SCr concentration are kept constant except for CrCl, then a 50% reduction in CrCl from baseline will lead eventually ( i.e. , at steady state) to a 100% increase in SCr, irrespective of the presence or absence of CKD. The steady state, however, may not be reached for several days after an episode of AKI. 3 , 4

will significantly delay the diagnosis of AKI in patients with CKD, because the level of baseline kidney function may influence the kinetics of creatinine rise after an acute injurious event to the kidney. CKD, which may affect >10% of the US population, 5 is a major risk factor for AKI and is present in >30% of patients who develop severe AKI. We therefore explored the implications of defining AKI by percentage versus absolute increases in patients with and without CKD in the time frames encountered in common clinical practice ( i.e. , before steady state is achieved). To do this, we solved differential equations that describe SCr kinetics after a change in CrCl at various levels of baseline kidney function. We propose that the definition of AKI should take into account the time course of the increase in SCr, and that early after injury absolute increases in SCr are superior to percentage increases because of the effects of CKD on creatinine kinetics.

The one-compartment and two-compartment Simulink models used for analyses are shown in Figure 1 . The change in SCr over time after an abrupt reduction in CrCl from 90 to 10 ml/min for 7 d followed by abrupt recovery back to 90 ml/min is shown in Figure 2 . After changes in CrCl, the two-compartment model exhibited a higher initial rate of change followed later by a slower rate of change compared with the one-compartment model. Overall, the differences between the one- and two-compartment models were minor.

Your child needs to see his or her doctor every 3 to 6 months. During these checkups, the doctor will evaluate and adjust your child's treatment. Diabetic ketoacidosis is almost always treated in a hospital, often in the intensive care unit, where caregivers can watch your child closely and give him or her frequent blood tests for glucose and electrolytes . Insulin is given through a vein (intravenous, or IV) to bring blood sugar levels down. Fluids are given through the IV to correct the electrolyte imbalance. Your child may stay in the hospital for a few days until blood sugar levels are back in the target range and electrolytes have normalized.

), the doctor may suggest a continuous glucose monitor (CGM). A CGM checks and records blood sugar day and night. Most CGMs sound an alarm if blood sugar levels are moving out of target range.

It’s the Holy Grail pursuit of brewing in hoppy beer making: year-round wet-hop flavor. Hops are good, fresh hops are better, wet hops are the best. We say ‘wet hops’ because they have not been dried after harvest. We say ‘better’ because they possess the fullest expression of hop flavor; vine-fresh. But as with another herbaceous favorite of ours, they must be quickly dried to prevent mold and spoilage. That drying process is done delicately but something is always lost in translation. Other good brewers have taken up the quest. The results have varied. Ours is a homegrown process of time dilation for the delicate hop cone that the flower doesn’t even perceive and so delivers its still newborn self to our kettle months and months after its birth. Questing has no end and we are still tweaking our process but we hope you find this mid-summer anachronism to be as satisfying as we do. If you dig it, enjoy it. We’ll make more! MORE ABOUT THE FREAKIN' WET HOPS: To figure out this brand new wet hop preservation process, we had to forget everything that we already knew about how hops are processed and preserved. We tried 5 or 6 different things to try and achieve this process (some of which failed miserably), then we discovered a relatively simple process that deliberately did all the things that the textbooks told us not to do. This new proprietary process involves a lot of time and money, and even more thinking about hops and freshness in new ways. From all of this, we found a brand-spankin’-new way to preserve the hops, fresh from the Yakima Valley trellises, first born during harvest season into the Born Yesterday last fall, and reborn into this scary new next step, the Born Again Yesterday Pale Ale, six months later, to the beer in your hands today.
